The Core Distinction
Iboga is the whole plant — Tabernanthe iboga, a shrub native to Central Africa, used for centuries in Bwiti initiation ceremonies. It contains 12+ alkaloids working in concert, with ibogaine as the primary psychoactive component.
Ibogaine is the isolated alkaloid — extracted and purified, used in clinical settings for addiction interruption, depression, and PTSD. The isolation changes the experience: more predictable, more measurable, less ceremonial.
Pharmacology
Ibogaine acts on multiple receptor systems simultaneously — NMDA antagonism, kappa opioid agonism, serotonin reuptake inhibition, and sigma receptor activity. This multi-target pharmacology is why it interrupts addiction at a neurobiological level that single-target drugs cannot reach.
The primary metabolite, noribogaine, has a half-life of 24-72 hours and is responsible for much of the anti-addictive effect. It persists long after the acute experience ends.
Cardiac Risk — Non-Negotiable
Ibogaine prolongs the QT interval. This is the primary safety concern. Deaths have occurred from cardiac arrhythmia in unsupervised settings. Required pre-treatment screening: 12-lead EKG, electrolyte panel (potassium, magnesium), liver function tests, full medication review.
Dangerous interactions: opioids, SSRIs, MAOIs, stimulants, antiarrhythmics, and many other medications. A complete medication washout protocol is required.
